Root Cause Analysis

Root Cause Analysis (RCA) is a systematic process used to identify the underlying causes of an event or problem. In aviation, RCA is used to investigate incidents, accidents, and other safety-related events. The goal of RCA is to identify the root cause(s) of the event, so that corrective actions can be taken to prevent similar events from occurring in the future. RCA involves gathering and analyzing data, identifying contributing factors, and determining the underlying causes of the event. It is an important tool for improving safety in aviation.
